Dear Lucky Duck,
Firstly: challenge accepted! 
Secondly, what would constitute a worse performance, I myself would go for more turns at 0% or more dice at 0%.
By the way you mentioned how many dice your opponent threw, but not how many you threw.
Topping either one would be a feat of unluck in itself.

In all fairness, the game was 9 turns between the two players, so you had only 4 turns ... you only rolled 11 dice, most or all of them were against tanks ... and on the last turn you moved an artillery with a Recon-1 instead of playing TFH.
The odds of missing a tank with 11 shots are only about 1 in 86. Pathetic, but not even on the Richter scale, compared to some of the other reports on this thread.
However, I do like the German tank on turn 3 moving through the mountain and directly to level ground. (Someone should tell the author of that scenario that mountains aren't supported yet.)

One more engagement which - apparently, could have, should have been taken ( TFH ) and we'd be talking about a 20% hit rate... Hardly ~ Special Medal ~ worthy IMO...

That's what I call a total missed throw : 16 dices against tank without any shot !! I claim your medal !!  
The more funny is that this game was totaly calamitous for both side. Lot missed dices during all the game !

You are certainly very close from the truth. In the box is the kid I stayed - who like a lot games (strategy games for sure !) - even if I don't look like anymore when I'm looking myself in a mirror

Hmmm, good points. I was thinking that the worst luck comes from a combination of bad dice AND bad cards. That's why I considered losing a six medal battle on turn 9 vs. the AI with a 0% hit rate the worst luck ever. I hadn't considered that I only rolled 11 times. Missing for 11 straight rolls is certainly within the realm of reasonable - reasonably unlucky that is - but not unheard of.
Having recently enjoyed a run of the other kind of luck - the good kind - and been promoted to Brigadier in the process - I feel there are many players out there more deserving of the 'Heart of Darkness' medal than yours truly. Therefore, I pass it on to Dietrich Von Kliest - if for no other reason than he may find a way to pretty it up a bit and add it to his rack of other medals and ribbons. That is something I'd like to see.

May I suggest this design?
It's based on the German medal for injuries (Verwundetenabzeichen).
The black heart is pierced by 2 swords and entangled in golden barbed wire. For a symbol of bad luck this can count I suppose.
And the ribbon that comes with it:
